Online Request Forms
Students who have the Buffett Scholarship are required to complete a request form in order to Study Abroad, place the scholarship On Hold, enroll in Less than 9 Hours, attend a Secondary College and/or Transfer to a different college. The request forms can be completed and submitted online.
Please click on the link below and the form will open in a new window. Once you complete each section and hit Submit, the form will be sent to the Foundation and you will receive a response within 5-10 business days.
Note, these forms are for Buffett Scholars only. We will not respond to forms received from students not on scholarship.
